If want to participate in the King’s All Star games, please fill out and submit this form before May 3st. If you do not submit, you will not participate. Entree fee is $2 a person. The player can play to a maximum of 2 games. Bring all payments the day prior to the first day of the games. You will receive your Game gear at that point in time. Player Sign Up Sheet
